Whistleblower Policy & Reporting

Our Commitment to Integrity

Quality Comprehensive Health Center (QCHC) is committed to the highest standards of ethical conduct, transparency, and accountability. We encourage employees, contractors, volunteers, patients, vendors, and community partners to report concerns regarding unethical behavior, financial misconduct, fraud, abuse, violations of law, or organizational policies.

QCHC is dedicated to fostering an environment where concerns can be raised without fear of retaliation.

What Should Be Reported

You are encouraged to report any activity that may involve:

  • Fraud, theft, or financial misconduct

  • Misuse of federal, state, or grant funds

  • Violations of healthcare laws or regulations

  • Billing or coding irregularities

  • Patient safety or quality-of-care concerns

  • Discrimination, harassment, or retaliation

  • Conflicts of interest

  • Violations of QCHC policies or ethical standards

Protection Against Retaliation

QCHC strictly prohibits retaliation against any individual who, in good faith, reports a concern or participates in an investigation. Retaliation against whistleblowers will result in disciplinary action, up to and including termination or severance of contractual relationships.

False or malicious reports made in bad faith may also be subject to disciplinary action.

How to Report a Concern

Reports may be made confidentially and, where permitted by law, anonymously.

Confidentiality & Investigation Process

All reports are taken seriously and reviewed promptly. QCHC will maintain confidentiality to the extent possible while conducting a thorough and impartial investigation. Information will only be shared with individuals who need to know in order to investigate and resolve the matter.

No Retaliation Guarantee

Anyone who believes they have experienced retaliation after reporting a concern should report it immediately using the same reporting channels. Retaliation claims will be investigated with the same level of seriousness and confidentiality.
