Our programs are designed to support individuals and families with access to resources, guidance, and ongoing case management.
Community Health Workers (CHWs)
Our dedicated Community Health Workers help patients navigate healthcare and community resources. They provide hands-on support for individuals receiving Medicaid and Medicare, ensuring access to the services, benefits, and care coordination needed to improve health outcomes.
Substance Abuse & Mental Health Case Management
Through our SAMHSA-funded BREATHE Program, we provide compassionate substance abuse and mental health case management. Our team connects clients to counseling, treatment options, recovery resources, and long-term support to promote stability and healing.
Peer Support for Substance Use
We believe recovery is stronger with community. Our Peer Support Specialists bring lived experience and training to help individuals struggling with substance use. By providing encouragement, mentorship, and guidance, they empower clients to stay engaged in treatment and maintain long-term recovery.
Ryan White Case Management
For individuals living with HIV, our Ryan White Program offers comprehensive case management, including access to medical care, prescription support, dental services, emergency housing, and other supportive resources. Our goal is to remove barriers so patients can receive holistic care that addresses both medical and social needs.
Housing Assistance through HOPWA
We understand that stable housing is critical to overall wellness. Through Housing Opportunities for Persons With AIDS (HOPWA) funding, we provide housing assistance, rental support, and resources to help clients achieve stability and independence.
