At Quality Comprehensive Health Center, we believe that cost should never be a barrier to receiving high-quality healthcare. Our Sliding Fee Discount Program (SFDP) is designed to make services affordable for patients based on household size and income.
What Is the Sliding Fee Program?
The Sliding Fee Discount Program offers reduced fees for eligible patients who are uninsured, underinsured, or experiencing financial hardship. Discounts are applied to services based on federal poverty guidelines.
Who Is Eligible?
You may qualify for discounted services if you:

Have a household income at or below 200% of the Federal Poverty Level (FPL)

Are uninsured or underinsured

Are willing to provide proof of income and household size
Eligibility is determined regardless of:

How the Program Works

Apply
Complete a Sliding Fee Discount application at the front desk or online.
2. Provide Documentation
Submit required documents, such as:

Recent pay stubs (last 30 days)

Tax return (most recent year)

Proof of unemployment or benefits (if applicable)
3. Determine Discount Level
Based on your income and household size, you will be assigned to a discount tier.
4. Receive Services
Your discount will be applied to eligible services at the time of your visit.
