What to Bring?
Medications List
Bring a list of your current medicines or bring your medicines with you.

Insurance Card
If you have insurance, bring your Medicare, Medicaid, private insurance, or other insurance information.

Photo ID
If your photo ID does not have your current address bring something to show where you live now.

Income Verification
Check stubs, Social Security income statement, unemployment benefits statement, W2, or current tax return.

Copy of medical records or names and contact information of places you have received medical care.

Services include: primary care & specialty referrals, onsite dental, HIV specialty care, hepatitis C treatment, lab/phlebotomy, women's health services, PrEP, hormone replacement therapy, nutritional counseling, mental health counseling, medication assistance, and case management.
