We're Glad You're Here!
At Quality Comprehensive Health Center, we want to make your first visit as smooth as possible. We ask for information to help you get the care you need at a price you can afford. You'll find it helpful to prepare some information before you come in for your appointment. It's a good idea to write down any questions you want to ask about your medical care so you don't forget. We're ready for most situations but let us know if you need special assistance like an interpreter. If you have questions or need to reschedule your appointment call us.
What to Expect
Your first visit is usually a consultation where you and your provider get to know each other. Follow-up appointments will be made for medical issues, annual physicals, or lab work. Specialty referrals will be made if needed.

Please download and fill out your new patient forms and bring them with you to your appointment. If you have no income, fill out the Income Statement form. You can fill these out on a computer and print them or print the blank forms to fill out by hand.
