Your Voice Matters

At Quality Comprehensive Health Center (QCHC), we are committed to providing respectful, compassionate, safe, and high-quality care. We value your feedback and want to know when your experience does not meet your expectations.

If you have a concern about the care, services, treatment, staff, or experience you received at QCHC, you have the right to submit a grievance.

What Is a Grievance?

A grievance is a concern or complaint about your experience with QCHC. You may submit a grievance regarding issues such as:

  • Quality of care or services

  • Staff conduct or professionalism

  • Communication

  • Access to services

  • Patient rights

  • Privacy or confidentiality concerns

  • Discrimination or harassment

  • Safety concerns

  • Appointments or scheduling

  • Facility or environmental concerns

  • Other concerns about your experience with QCHC

How to Submit a Grievance

You may submit a grievance using our Online Grievance Form.

You may provide your contact information if you would like someone from QCHC to follow up with you. You may also choose to submit your grievance anonymously.

What Happens After You Submit?

Your grievance will be reviewed by the appropriate QCHC personnel in accordance with our grievance policies and procedures.

Depending on the nature of your concern, QCHC may review relevant information, speak with appropriate staff, investigate the circumstances, and determine whether follow-up or corrective action is appropriate.

If you provide contact information, a QCHC representative may contact you for additional information or to communicate regarding your concern.

Confidentiality & Non-Retaliation

QCHC respects your privacy. Information submitted as part of a grievance will be handled as confidentially as reasonably possible and shared only as necessary to review and address the concern.

QCHC prohibits retaliation against individuals who raise concerns or submit grievances in good faith. Submitting a grievance will not affect your ability to receive services from QCHC.

Before Uploading Documents or Images

If you choose to provide supporting documents or images, please only upload information directly related to your grievance.

Do not upload medical records, medication information, laboratory or test results, insurance information, Social Security numbers, identification documents, or other sensitive personal or health information.

Please also make sure photographs do not show patient information, medical documents, computer screens containing confidential information, identification cards, or other individuals without their permission.

The Online Grievance Form is not intended for medical emergencies or situations requiring immediate assistance.

If you are experiencing a medical emergency or believe someone is in immediate danger, call 911 or seek emergency medical assistance.
